How does Preservé™ differ from a traditional breast augmentation?

In every breast augmentation, the plastic surgeon first creates a pocket: a custom-made space where the breast implant will be placed.

Classic breast augmentation

In a traditional breast augmentation, this pocket is created by cutting through breast tissue, supporting ligaments (such as the Cooper’s ligaments), and sometimes even the breast muscle (pectoralis major).

Placing the implant behind (under) the muscle is an outdated technique, now less commonly used, involving extensive cutting and therefore significant post-operative pain and a long recovery period of at least six weeks. Even with the more modern dual plane placement (partly behind the muscle, partly behind the breast tissue) or placement in front of the muscle, healthy breast tissue still needs to be cut to create the pocket.

Cutting breast tissue and muscles in a traditional augmentation carries risks such as increased pain, scarring, longer recovery, and possible implant displacement.

Preservé breast augmentation

The Preservé™ technique represents a true turning point in this regard, with the following differences compared to traditional breast augmentations:

1. How the pocket is created:

The pocket is no longer surgically cut but gently expanded using a Motiva® inflatable balloon. This keeps the ligaments and muscles intact, while surrounding tissue is carefully and evenly displaced to make space for the implant.

2. How the implant is inserted:

The implant is inserted using the Motiva® Insertion Sleeve with the no-touch technique. This sterile insertion sleeve allows the implant to be placed without direct contact with the skin, made possible by the flexibility of Motiva Ergonomix® implants.

Benefits: reduced risk of contamination or infection, smaller scars, and less trauma during insertion (therefore lower risk of complications such as capsular contracture and preservation of nipple sensitivity and breastfeeding ability).

3. How the implant is positioned:

With the Preservé™ technique, the implant is always placed in front of the breast muscle – a method Dr. Nelissen has successfully applied for years and fully masters. The major advantage is that the breast muscle is completely preserved, resulting in less (to no) post-operative pain and a very rapid recovery.

A frequently asked question is: “Doesn’t the implant sag faster this way?”
The answer is no, quite the opposite. During tissue expansion, the supporting ligaments tighten, creating a firm “nest” around the implant, providing better support and ensuring it stays stable in place. This is also one of the additional advantages of the Preservé™ breast augmentation.

How is a Preservé surgery performed?

The Preservé™ technique uses advanced, single-use surgical instruments – such as the Inflatable Balloon and the specialized Chanel Separator. These tools allow the internal pocket for the implant to be prepared with extreme precision and control, without cutting or damaging the breast tissue, as is the case with traditional breast augmentation.

For the Preservé™ technique, next-generation breast implants (SmoothSilk® Ergonomix2 Joy) are used. Their high flexibility allows them to be inserted through a minimal incision using a specialized sleeve.

The Preservé instruments

Motiva® Channel Separator

- for creating a tunnel (passage) through the smallest possible incision

With this single-use dissector instrument (Channel Separator), a passage or tunnel is gently created (“tunneling”) through a minimal incision (+/- 2.5 cm) in the fold under the breast, reaching the opposite edge of the breast pocket, allowing the inflatable balloon to be inserted. The blunt tip of this instrument carefully separates the internal breast tissue using a circular twisting motion. This technique is designed to minimize tissue trauma and preserve the breast’s natural support system.

Next, another single-use instrument is inserted through the created tunnel: the Inflatable Balloon. By inflating this balloon, the breast pocket (the space for the implant) is carefully formed. The breast tissue is gently displaced, and the ligaments are stretched rather than cut.

This technique minimizes damage to the natural breast tissue (resulting in less pain, faster recovery, and fewer complications) while firmly engaging the supporting breast ligaments and muscles, increasing the stability and support of the implant.

Motiva® Insertion Sleeve

- for the no-touch insertion of the breast implant through the small incision

Using the Motiva® Insertion Sleeve, the highly flexible Motiva Ergonomix2® implant is placed efficiently and safely through a minimal 2-centimeter incision (depending on implant size). This allows patients to benefit from a faster recovery and virtually invisible scars. The no-touch technique also minimizes the risk of infection.

This less invasive* breast augmentation technique minimizes damage to the natural breast structures and the major breast muscle (pectoralis major). Preservé™ gently stretches the existing tissue to form a “nest” or “basket” around the breast implant. This approach enhances stability and support, as demonstrated in a clinical study reporting 0% chance of bottoming out after three years.
